Your physician has recommended Physical Therapy/Occupational Therapy and there are a few things you should know prior to your first appointment. First of all, you have the right to choose where you would like to go for your therapy services. You may use the prescription for therapy provided by your physician at any location of your choosing in your local community.
Please be sure that you do have an up-to-date referral/ prescription from your doctor with you, or confirm that your physician has sent the referral to our facility prior to arrival. This is necessary for us to perform the initial evaluation.
Although we can assist in determining your coverage with your insurance plan, it is always wise to call your insurance company, yourself, prior to your first visit, to determine what coverage that you actually do have for physical therapy/occupational therapy services.
Aspirus Keweenaw Rehabilitation Center is a participating provider with most major insurance plans including Medicare, Medicaid, UPHP, Blue Cross/Blue Shield, auto insurance, private insurance carriers, etc. Please check with the front desk when you make an appointment to verify that we are, in fact, a participating provider.
Please arrive 15 minutes prior to your scheduled appointment time to allow us time to process your paperwork.
Wear loose, comfortable clothing that will allow your injured body part to be exposed and evaluated by your therapist.
Please plan on spending at least one hour on your first appointment. This allows the therapist to spend time evaluating you and establishing a treatment plan to allow you to attain your goals.
Regarding your attendance, it is extremely important that you try to make all of your appointments, and become personally involved in your care plan. This means that you will be provided with a home program, that you will be expected to carry out on a daily basis as instructed by your therapist. If you cannot make an appointment, please call at least 24 hours in advance to cancel and/or change your appointment times. For those patients who have at least three no shows, you will be discharged from physical therapy and will be required to obtain another prescription prior to resuming your therapy services. When you register for your first appointment, you will be provided with a booklet titled, “Guide To Outpatient Rehabilitation Services”, and we recommend that you read through this booklet at your convenience.
